The form will be submitted to the registrar for processing.   Also, if you are a new student you will need to submit three references to be admitted.  You can do this online as well.  After you submit this form, the next page will direct you to an online form you can send to your potential reference. Note that the name, address, email, and student photo upload is required to submit this form.
