Instructions for Submission of Scholarship Application

Mail/deliver the original and one (1) additional copy of this application and all attachments to:

The scholarship committee will review the applications and determine the scholarship amount(s) to be awarded to each student winner. The awardee(s) will receive a notice of scholarship certificate via mail prior to the start of the corresponding semester.
The funds from the scholarship will be dispensed to the winner once the following conditions are met:
1. The student is enrolled and on campus at the institution of higher learning.
2. The student’s schedule and semester cost have been established and received by the student.
3. A “Thank You” letter from the recipient to the scholarship donor.
